What to include
- The requested make, model or approved specification, copied from your institution's requirement.
- Quantity for each unit, accessory and consumable line.
- Facility name, receiving department, delivery address and city.
- Requested start date. Mark it provisional if it may change.
- Institutional contact name, role, work email and phone number.
- Accessories and consumables on separate lines, with a quantity for each item.
Do not include patient names or health information.
